Call for examples of good practice in home care for older people

SCIE is producing new guidance and looking for case studies. Would be great to see some co-production examples in there.............please respond to the contact listed below! SCIE is producing a Department of Health-funded accredited digital guide on home care for older people with complex needs. The project, guided by an advisory group of service users, carers, providers and commissioners, will draw on published research evidence as well as practice examples. We want to hear about dignified, personalised home care which empowers older people with complex needs; those who are frail, those who have chronic health conditions, mental health issues and/or difficult social circumstances. If you know of an exemplary service, please complete a short questionnaire. Your responses will be considered for inclusion in the guide which will be published in early summer 2014.  We will check the information to be used before publication. Thank you very much.
