Following a collaborative process, which included an evidence review from Newcastle University, over 100 online responses and input from relevant academics, representatives from the wider research community and leading practitioners and decision-makers in the field of person-centred and community-based care, 'Realising the Value' have established five areas of practice for the programme.

We've launched an open call to become a local partner site to Realising the Value.
We're looking for organisations or partnerships with significant experience working with a person-centred and community-based ethos in one of our above areas of practice and an interest in helping others on the way.
Becoming a local partner offers you the opportunity to showcase your work nationally and to work with our consortium to: evidence the impact of person-centred care and community-based approaches, identify any barriers to implementation and help to shape tools and recommendations for the wider health and care system as part of theNHS Five Year Forward View.
We offer a small grant to cover the direct costs of involvement. In return, we ask for your leadership and commitment to shaping high quality tools and resources for others to use. For more information please see our call for applications. You can apply online until 3pm, 13 October 2015.
We've published our first discussion paper, setting out our initial thoughts on a new articulation of value. This is part of our overall mission to demonstrate the value of people and communities being involved in their own health and care.
We're asking people to consider the following questions:

Do you work in one of our five areas of practice and have you used or developed toolkits or learning materials to implement your work? Then we would like to hear from you.
Over the coming months the Realising the Value team, with the help of local partners, will be producing tools and resources for commissioners, practitioners and organisations that can equip the health and care system to take up high impact person-centred care practice at scale - for example, by developing tools to support practitioners and organisations to introduce new ways of working.
We are aware that many organisations have already produced materials and toolkits aiming to spread adoption and good practice. We want to learn from and build on them. This is a great opportunity for your work to be nationally recognised and shared.
